Nosocomial infection in Hepatobiliary Surgery : An Investigation and Analysis of 82 Cases

【Abstract】Objective To investigate the characteristics and risk factors of nosocomial infection in hepatobiliary surgery, find the effective measures to reduce nosocomial infection. Methods The inpatient cases of hepatobiliary surgery from January to December of 2010 were retrospecvtively surveyed. Results 82 people and 105 cases were infected, the infection rate was 2.54%, case infection rate was 3.25%. Infection types of surgical site infection (41.90%), lower respiratory tract infection (26.67%) and blood stream infection (16.19 %). Of the isolated strains, the rates of bacterial were gram-negative bacteria (62.61%), gram-positive bacteria (30.43%) and fungi (6.96%). The patients after liver and pancreatic tumor resection with the highest postoperative infection rate (39.39% and 15.49%). Conclusion The hospital infection surveillance and rational use of antimicrobial drugs, improve the operation plan and shorten the operation time, strict aseptic technique, strengthening the management, and enhance awareness of medical staff hand hygiene is the important measures to reduce nosocomal infection in hepatobiliary surgery .

【Abstract】 Objective To explore the methods for diagnosis and prevention of complications of pancreatic injury. Method The Clinical data of 29 cases of pancreatic injury were analyzed retrospectively.Result The conformity diagnosis rate of CT was 76.2%. Five cases received nonoperative treatment including 3 cases of grade I and 2 of grade II injury. Pancreatic pseudocyst occurred in 2 patients,and were cured by Cyst drainage after 3 months.A total of 24 cases with pancreatic injury underwent operation. Among the 24 cases,22 patients were cured and two patients died of multiple organ failure. pancreatic fistula and varying degrees of abdominal infection occurred in 9 patients, 2 cases of abdominal bleeding, 3 cases of pancreatic pseudocyst ,no abdominal abscess.Conclusion Early diagnosis and timely management for pancreatic injury and choosing reansonable treatments can improve the curative effect and reduce the complication effectively of pancreatic injury.

Abstract objective Observing the clinical effect of Triple drug therapy (orally, wash-out, rectally) on aged patient with incarcerated hemorrhoid. Method taking tablet Di Ao Si Ming, hip baths with POWDERS JIN XUAN ZHI KE and applying MA YING LONG HEMORRHOIDS OINTMENT rectally to treat 61 aged patients with incarcerated hemorrhoid, the course of treatment last two weeks, analyzing the clinical effect at the 7th day and 15th day. Results the total effect rate at the 7th day is 96.72%,it is 100% at the 15th day . Conclusions the triple drug therapy to treat aged patients with incarcerated hemorrhoid is easy, with fast and good effect, easily received by patient and deserved spreading

Objective to study the sentinel lymph node biopsy neoadjuvant chemotherapy for breast cancer after value of clinical application. Methods from September 2007 November 2010 during the Ⅱ 46 cases treated patients with breast cancer, Ⅲ period after neoadjuvant chemotherapy axillary lymph node ultrasound negative person as the research object. 46 patients sentinel lymph node biopsy axillary lymph node cleaning the subtotal standards. All the sentinel lymph node do pathology and immunohistochemical inspection. All the sentinel lymph node do regular pathological examination. Results the sentinel lymph node biopsy success rate, false negative rate, sensitivity, specificity and accuracy, respectively, negatie predictie positie predictie reprinted, 3.7%, 96.3%, 100%, nine, 100%, sentinel lymph node biopsy 94.1%. Conclusion with ultrasound can accurate evaluation neoadjuvant chemotherapy axillary lymph node status after breast cancer patients to decide whether you need, do armpit lymph node dissection have certain reference value.

【Abstract】 Objective To measure the setup-error in the intensity modulated radiation therapy after breast-conserving operation using the Thorawedge breast bracket. Methods Patients of 10 cases were applied with the Thorawedge breast bracket for positioning and intensity modulated radiation therapy after breast-conserving operation. Technicians set up with the setup signs and screened using the electronic portal imaging device (EPID), and the error was analyzed through comparing the portal film with the digitally reconstructed radiographs (DRR) in the MOSAIQ system. Result The setup errors were (3.29±1.58)mm in left-right direction,(2.38±1.36)mm in cranial-caudal direction,(3.51±3.05)mm in anterior-posterior direction. The percentage of the setup errors exceed 5mm were 18.8%,12.5%,31.3%in three directions. Conclusion There exists some extent of setup error in the intensity modulated radiation therapy after breast-conserving operation using the Thorawedge breast bracket. We can use EPID on-line to correct the setup error in order to improve the setup precision.

Objective There is an uncommon subset of supracondylar humeral fractures in children that are so unstable they can displace into both flexion and extension. The purposes of this study were to describe this subset of supracondylar fractures and to report a technique of opened reduction and percutaneous pinning for their treatment.Methods In a review of 8 severely displaced displaced supracondylar humeral fractures in children treated operatively at our institution, we identified eight that were completely unstable with documented displacement into both flexion and extension as seen on fluoroscopic examination with the patient under anesthesia. We used a technique for opened reduction and fixation of these fractures, and then we assessed fracture-healing and complications from the injury and treatment.Results All eight fractures were treated satisfactorily with opened reduction and percutaneous pinning. None of the patients had a nonunion, cubitus varus, malunion, additional surgery, or loss of motion.Conclusions In severely displaced supracondylar fractures in children, multidirectional instability results in displacement into flexion and/or extension. This fracture may be classified as type IV according to the Gartland system, Severely displaced supraacondylar f racture of the humerus still needs open reduction These fractures can be treated successfully with a technique of opened reduction and percutaneous pinning. hard intermal and external fixation ,early elbow joint functional training are the important measures 。

Hypoxia is very common in solid tumor. Hypoxia-inducible factor-1α(HIF-1α) is the Transcription factor generally existed in mammal and human cells. Many hypoxia-responding genes can be activated by HIF-1α. It can facilitate malignant cells invasion and metastasis through affecting tumor cell’s capability of adhesion and migration, the degradation of extracellular matrix(ECM). Depressing the expression level of HIF-1α may inhibit malignant biological behaviour of tumor, thereby improving patient’s poor prognosis.
