    【Abstract】〓Scoliosis types include functional scoliosis and structural scoliosis in that idiopathic scoliosis account for about 70-80% of all and are the most common structural clinically. Domestic and abroad research shows that prevalence rate of adolescent idiopathic scoliosis (AIS) is about 0.5%-3%, and it is believed the percent would rise with the pupillary study burden to raise in the present Chinese education. The key of prognosis of scoliosis is early detection, early intervention. The most effective way for early discovery of AIS should be census. Because the development of scoliosis often occurred in the period of rapid growth, in which AIS is easy to be corrected.

    Clinical experience with laparoscopic transabdominal preperitoneal hernioplasty in treatment of inguinal hernia

    【Abstract】 Objective〓To summarize the 64 patients with inguinal hernia were performed the laparoscopic transabdominal preperitoneal (TAPP) approach, and explore its application skills and value. Methods〓To collect and analysis 64 patients performed transabdominal preperitoneal(TAPP) with inguinal hernia between April 2010 and April 2015. Results〓The operation were successfully completed, and the time of operation were 85±10.6 min (68-108 min), average bleeding volume were 35.25 ml, hospital stay after operation were (5.6±0.8)d (3-9 d). Three cases of scrotal edema, one case of seroma,three cases of acute urinary retention occurred after operation with 10.94% of complication rate,and all were cured. A mild pain occurred in post-operative patients,without incision swell, wound extravasating and none of patients received analgesics. Cases were followed up for 2-60 months with an average of 19 months and 92.19% of follow-up rate. Five cases failed to be followed up. No recurrence was found. Conclusion〓TAPP is a reliable efficient technique with shorter hospitalization and less complication to repair adult hernia or direct hernia or femoral hernia, especially to the hernia recurrence and incarcerated hernia with intestinal obstruction.

    【Abstract】〓Objective〓To evaluate the safety and efficacy of pre-operative chemoradiation, using capecitabine and bevacizumab with standard doses of radiation, in patients with locally advanced rectal cancer. Methods〓Patients with locally advanced cancer were treated with capecitabine 825 mg/m2 twice daily on days 1-14 and bevacizumab 5 mg/kg on days 14, 1, 15 and 29, and radiation 50.4 Gy in 28 fractions including boost.Total mesorectal excision was performed 7-9 weeks after chemoradiation. Results〓Forty-two evaluable patients were enrolled, and 38 proceeded to definitive surgery. Eighteen patients (43%) had clinical T4 tumours and/or N2 tumours. Mean relative dose intensity was >90% for all systemic agents, and 97% for radiation. Grade 3/4 diarrhoea occurred in 10 patients (24%) and pain in 4 patients (10%) pre-operatively, while grade 3/4 pain, fatigue and infection were each reported among 5 patients (13%) post-operatively. Re-operation due to complications occurred in 4 patients (11%). Complete tumour regression (ypT0) was seen in 9 patients (23.7%) of which 2 had N1 disease (5.3%) and the pathological complete response (pCR) rate (ypT0N0) was 18.4%. Conclusion〓In this study, pre-operative bevacizumab added to capecitabine and radiation was safe and resulted in a promising tumour regression rate.

    【Abstract】〓Objective〓To evaluate and summarize the effect of surgery on displaced acetabular fractures. Methods〓From May 2010 to May 2015, 19 cases of displaced acetabular fractures were treated in our hospital. Case files were retrospectively analyzed. Results〓Through follow-up from an half to two years (average 1.5 years), the clinical effect in 19 cases was evaluated by the improved d’Aubigne and Postel hip score standards and results included excellent in 14 cases, good in 3 cases, fair in 1 cases, poor in 1 cases and a good rete of 89.5%. One patient was performed artificial total hip replacement due to post-operative severe traumatic arthritis and ischemic necrosis of femoral head. One patient developed heterotopic ossification. And one patient was injured sciatic nerve. All three patients were getting well. Conclusion〓Proper surgical treatment for displaced acetabular fractures is appropriate for reconstruction of the congruency and stability of the hip joint and allows the patient to do weight-bearing earlier.

    【Abstract】....Objective...To discuss the role of the mean arterial pressure (MAP) and pulse pressure (PP) in early assessment on vascular crisis after wrist replantation. Methods〓A retrospective clinical data were analyzed in 8 cases from August 2014 to November 2015, who suffered from vessel crisis after wrist replantation (observation group), and 10 cases with stable vital signs in ICU as control group. Both groups received continuous brachial arterial pressure. MAP and PP were recorded at 2 h, 1 h, and 0.5 h before vessel crisis happened. All cases with vascular crisis were confirmed by digital subtraction angiography (DSA). Results〓All 8 cases with vessel crisis were handled in time and performed extremities survived after surgery. The values of MAP and PP (mmHg) recorded at 2 h,1 h,0.5 h were 70.13±4.86 and 46.88±4.84,62.00±4.48 and 51.88±4.27,66.25±1.51 and 66.13±1.16, respectively. The values of MAP and PP (mmHg) in control group were 72.5±3.3,46.4±2.8. However, only the value of PP at 0.5 h showed the significant statistical significance. Conclusion〓PP may be as an early predictor of vascular crisis after wrist replantation. However, the samples in this study was limited, as well as the measuring method of MAP and the design, which should be improved.

    【Abstract】〓Objective〓To evaluate the applicability and experience of retroperitoneal laparoscopic operation in primary hospital. Methods〓The clinical data of 81 patients that underwent urologic surgery by retroperitoneal laparoscopy were retrospectively collected and analyzed. Results〓The surgeries were successful in 79 cases. One case of adrenal neoplasm was converted to open surgery because of heavy bleeding during operation. Another case of renal cyst was converted to open surgery due to active bleeding post operation. The average time for operation was 2.5 h (ranged 0.5-7.5 h). No severe complication was observed after operation. The average hospitalized time post-operation was 5.5 d (ranged 2-14 d). Conclusion〓Retroperitoneal laparoscopy is a suitable choice for urologic operations in primary hospitals, with the advantages of minimal invasion, rapid recovery, less complication and short hospitalization days. The step-wise progression from basic to advanced techniques, the optimal screening of candidate through indications and contraindications, and pro-active problem solving is essential at the initial stage.

    【Abstract】〓Objective〓To investigate the expression of PTEN and Beclin1 in bladder cancer patients and to evaluate its clinical significance. Methods〓Immunohistochemical method was used to determine the expression of PTEN and Beclin1 in 65 cases of bladder cancer patients and the para-carcinoma tissues, and another 15 cases of normal bladder tissues. The relationship between PTEN and Beclin1 in cancer tissue was analyzed by Spearman correlation analysis. Results〓PTEN expression rate in cancer tissues,adjacent tissues and normal tissues were 53.8%,81.5% and 93.3%, respectively. Beclin1 high expression rate in cancer tissues,adjacent tissues and normal tissues were 58.5%, 84.6%and 86.7%, respectively. PTEN and Beclin1 high expression rate in cancer tissues were significantly higher than that of adjacent tissue and normal tissue (all P values were more than 0.05). PTEN and Beclin1 high expression rate in cancer tissues were correlated with histological grade and clinical staging of patients, regardless of patient gender and age. Beclin1 and PTEN expression decreased along with histological grade and clinical staging of patients increased, respectively (all P values were less than 0.05). Beclin1 expression was positively correlated with PTEN expression in cancer tissues (P<0.001). Conclusion〓The expression of PTEN and Beclin1 in bladder cancer were associated with tumor occurrence, progression.

    【Abstract】〓Objective〓To study the expression and significances of IMPDH2 in benign prostate hyperplasia (BPH) tissues and prostate cancer tissues, and to illustrate the correlation with clinical significance. Methods〓The samples were derived from benign prostatic tissue and prostatic carcinoma tissue of the patients with BPH or prostatic cancer. Western Blot was used to detect IMPDH2 protein in prostate cancer tissue and BPH tissue. Immunohistochemical method was used to detect IMPDH2 in prostate cancer tissue and BPH tissue,and evaluate the correlations with clinical significances of prostate cancer. Results〓The expression of IMPDH2 protein was significantly increased in prostate cancer tissues. The expression rate of IMPDH2 was in relation with clinical stage, Gleason score, and metastasis. Conclusion〓Our study showed the expression of IMPDH2 protein has positive correlation changed trend, suggesting IMPDH2 protein detection may help judge pathologic grade and predict the prognosis of prostate cancer.

    【Abstract】 Objective To study the mechanism of high migration rate group protein B1(HMGB1) migration mediated autophagy protects the liver cells injury in sepsis. Methods〓Seventy-two male Lewis rats were randomly divided into endotoxin group (n=18), autophagy induction group (n=18), autophagy inhibition group (n=18) and control group (n=18). The rats in endotoxin group was given intraperitoneal injection of endotoxin (LPS, 25 mg/kg) to induce endotoxemia. Rats in autophagy induction group and inhibition group received intraperitoneal administration of rapamycin or ward penicillin pre stimulation respectively, and followed establishment of endotoxemia one hour later. Rats in blank group were treated with intraperitoneal injection of Ringer's solution(1 ml). The changes of HMGB1 and autophagy in liver tissue, liver function and cytokines were observed on the time points of 6 h, 24 h, 48 h after operation. Results〓In the endotoxin group, the autophagy of hepatocytes, the HMGB-1 in hepatocytes cytoplasm,the liver enzymes in serum and IL-1β levels increased significantly (P<0.01). However, the level of HMGB1, liver enzymes and IL-1β in the rats of autophagy induction group significantly decreased (P<0.01) compared with the endotoxin group. Conclusion〓In the sepsis, HMGB1 migration mediated autophagy regulates in IL-1β to reduce liver cell injury and promote the recovery of liver function.

    【Abstract】Objective To investigate the cardiovascular response and extubation time of severe obese patients undergoing tracheal intubation by using lean body weight(LBW) as a criterion to calculate intravenous induction dose. Methods Thirty patients, ASA I~II, aged from 18 to 60 years old, BMI>40, who were scheduled to perform general and orthopedic surgery undergoing general anesthesia,were involved in this study. Patients who were given anesthetics according to their lean body weight (LBW), total body weight (TBW) and ideal body weight (IBW), were assigned into 3 groups:group LBW, groupTBW and groupIBW respectively. The average of three record of systolic pressure (SBP), diastolic pressure (DBP) and heart rate (HR) were taken as base values (T1). The SBP, DBP and HR of patients in three groups at the moment just before intubation (T2) and at the moment intubation (T3) as well as extubation time (time from the end of surgery to extubation) were recorded. Results (1) The decreases of SBP, DBP and HR in groupTBW at T2 were much more than that in groupLBW and groupIBW compared with T1 (P<0.01). (2) The decreases of SBP, DBP and HR in groupTBW were much more significant than that in groupLBW at T3 compared with T1 (P <0.01). While those were increased over 15% in groupIBW compared with T1(P<0.01). (3)The incidence of SBP>30% baseline, HR>100/min and RPP>12000 were highest in groupIBW(60%,50%, 80%, P<0.01), the incidence of SBP<30% baseline and HR<60/min were highest in group TBW(70%, 50%, P<0.01). (4) Extubation time in group TBW was longest (26.2±12.5 min, P<0.01) among the three groups. Conclusion LBM may be considered as a reasonable criterion for ASA Ⅰ~Ⅱ, severe obese patients to calculate the induction dose of intravenous anesthetics since it has less impact on cardiovascular reaction,lower incidence of adverse reaction and faster recovery from general anesthesia.

    Malignant mesothelioma is a class of rare malignant tumor with a poor prognosis, which originates in the serosal surface. Due to the insidious onset and nonspecific clinical manifestation, it is not unusual for the missed diagnosis of malignant mesothelioma clinically. Pathology and immunohistological techniques have so far been the major diagnostic approach, owning the high sensitivity and specificity. This article reported a case of colon cancer complicated with malignant peritoneal mesothelioma, and summarized a literature review about it's pathogenesisas, diagnosis and treatment as well, trying to promote the clinical cognition of the disease.

    【Abstract】〓Objective〓To invesgate regional anatomy features of related vessels and surgical planes in posterior approach laparoscopic pancreaticoduodenectomy (paLPD). Methods〓In order to distinguish related vessels in paLPD accurately, we explored anatomy symbols, exposure methods and search for safe surgical planes under laparoscopic visions by means of analysis from 25 patients surgery videos and observing anatomy features of 15 adult specimens. Results〓Four regions are identified as important surgical planes in paLPD,including the avascular region between posterior pancreas duodenum and prerenal fascia,the avascular region between posterior pancreatic neck and superior mesenteric vein, the region between pancreatic uncinate process and mesenteric vessels, and the celiac trunk and hepatoduodenal ligament regions,and they cover all related vessels in paLPD. Operating along accurate surgical planes,we can not only reduce vessels injury risks,but also improve surgery efficiency and follow the “en bloc” principle better. Conclusion〓paPLD is a safety and feasible way in LPD,and we can improve surgery safety and efficiency by mastering the features of related vessels under laparoscopic visions and operating along accurate surgical planes.

    【Abstract】〓Objective〓To investigate the efficacy of surgical glue in treatment of facial wounds in emergency surgery. Methods〓One hundred patients with facial wounds whom were treated in our Emergency Department from 2012 Jan to 2013 Dec were chosen in this study. They were randomly divided into two groups (50 patients each group), surgical glue treatment group and the conventional surgical debridement group. The clinical efficacy was compared between two groups. Results〓Compared with conventional surgical debridement group, surgical glue showed advantages in safety, convenience and debridement time, and cosmetic effect in treatment with small facial wounds. Conclusion〓Surgical glue is more effective than conventional debridement in treating facial wound, but is used in eligible patients.

Results〓The age in the cases was 35,.28,.30 years old,.respective..All presented hypertension during pregnancy. The level of catecholamine or its metabolites of plasma or urinary in patients were higher than normal reference values..B ultrasound and MRI found adrenal tumor in the three patients. One case was found clinical silent adrenal tumor before pregnancy,.and presented hormonal activity after pregnancy..Another case presented pneumonedema when cesarean section..These two patients accepted laparoscopic surgery of adrenal laparoscopic pheochromocytoma. The histopathological findings proved pheochromocytoma..The third case underwent no surgical treatment..Conclusion〓Pheochromocytoma in pregnant patient is one of the most threatening medical conditions for mother and fetus. Early awareness and recognition is very important, the treatment of clinical silent adrenal pheochromocytoma in reproductive women should be more active.

    【Abstract】 With the wide popularity of B-ultrasonography imaging technology,.PLG detection rate is higher and higher, has become a common disease of biliary surgery. Currently on PLG treatment is mainly surgery, and so far no effective way to identify the nature of PLG before the surgery, thus makes the most patients without malignant change tendency of PLG imposed unnecessary cholecystectomy. In view of this, this article will to review the relationship of PLG and malignant transformation, and to understand the risk factors of PLG and its malignant transformation, so as to determine surgical indications of prophylactic cholecystectomy has a certain clinical significance.

The patients in the therapeutic group were treated with potassium hypermanganate and hirudoid and those in the control group were treated with potassium hypermanganate and compound carraghenates. The difference in effective rate between two groups was compared. Results〓The effective rate of therapeutic group was 86.8%, control group was 65.7%. The difference between two groups was significant. Conclusion〓Hirudoid is effective in treatment thrombosed hemorrhoids.

    【Abstract】 Objective To explore the feasibility and effect of unilateral thyroid operations by subclavicular oblique incision. Methods A total of 359 cases of unilateral thyroid operations performed between Jan 2006 and Dec 2011 were retrospectively analyzed..Pre-,.intra-and post- operative information were compared. All patients were divided into three groups based on incision types for thyroidectomy, low-collar incision (LI group, n=110), supraclavicular oblique incision (Sup-I group, n=159) and subclavicular oblique incision (Sub-I group, n=90). Results All of operations in three groups were accomplished successfully,.and the patients healed by first intention..Among the 3-24 months of follow-up, no case of tumor residual and recurrence were found..There was no significant difference of mean incision length, operating time and blood loss. One case of recurrent laryngeal nerve injury occurred in each group, with insignificant difference in statistics analysis..Scar adhesion occurrences were 10, 1, 0 respectively in LI, sup-I and sub-I group respectively,.with significant difference in three groups (P<0.001). The appearance was more satisfactory in the Sub-I group (P<0.001)..Conclusion The unilateral thyroid surgery by subclavicular oblique incision has the advantage of safety, lower complications, cosmetic result and is worth for clinical expansion.

    【Abstract】 Objective〓To investigate the predictable factors of metastasis in non sentinel lymph nodes (NSLN) in patients with prime breast cancer with positive sentinel lymph nodes (SLN). Methods〓Patients with primary breast cancer and positive sentinel lymph node (n=212) were included in this retrospective study.All patients had positive SLNs that had been ordered by the concentration of tracer,and were analyzed to determine factors that predicted additional positive axillary nodes. Pearson Chi-square or T-test were used for univariate analysis. Logistic regression was used for multivariate analysis..Results〓Of 212 breast cancer patients with positive sentinel lymph node ranked by the concentration of tracer, multivariate analysis revealed that NSLN was associated with the SLN metastases rate (P<0.001), the number of negative SLN (P=0.02), metastases in the latest lymph node (P=0.02) and the size of SLN metastases (P<0.001) which independently predicted the NSLN metastases. Conclusion〓SLNs with higher concentration of tracer might have higher possibility of metastasis. Our study suggested that the SLN metastases rate, the number of negative SLN, metastases in the latest lymph node and the size of SLN metastases independently associated with NSLN metastasis.

    【Abstract】〓Objective〓To construct, package and identify the recombinant lentivirus overexpressing hepatocyte nuclear factor 4 alpha, HNF4A, and to lay the foundation for further study the functions and mechanism of action of HNF4A. Methods〓The human HNF4A cDNA was amplified from the human genomic DNA by polymerase chain reaction (PCR), which was constructed to the lentiviral expression vector PCDH-CMV-MCS-EF1a-GFP-puro. After enzymatic digestion and sequencing,the positive recombinant PCDH-CMV-HNF4A-EF1a-GFP- puro expression vector, pCMV-VSV-G and pCMV-dR8.91, three plasmids were cotransfected into HEK-293T cells, next the supernatants were harvested. After getting the virus suspension concentrated, we make the gradient dilution to infect HEK-293T cells and determine the virus titer by the green fluorescence protein. Finally the expression of HNF4A in primary cultured human skin fibroblasts HFFs infected with recombinant lentivirus was analyzed by real-time quantitative PCR. Results〓Enzymatic digestion and DNA sequencing demonstrated that the recombinant lentiviral expression vector PCDH-CMV- HNF4A-EF1a-GFP-puro was successfully constructed. The lentivirus, with the concentration of 1×108 TU/ml, can significantly increased the level of HNF4A expression in HFFs which was infected. Conclusion〓The recombinant lentivirus with PCDH-CMV- HNF4A-EF1a-GFP-puro was successfully constructed and highly overexpresses HNF4A in HFFs, what make a contribution to the better carrier for HNF4A with high efficiency and stability.

    【Abstract】〓Objective〓To investigate the role of early enteral nutrition in severe head injury patients with stress hyperglycemia. Methods〓Sixty cases of severe head injury complicated with stress hyperglycemia were recruited and a retrospective analysis from these patients was performed. According to the given enteral nutrition time,.the patients were divided into early enteral nutrition group (beginning from 8 hours after admission, n=30) and delayed enteral nutrition group (beginning from 48 hours after admission,.n=30)..Quality in evaluating diabetes control, hypoglycemia, and complications were observed and analyzed. Results〓Compared with delayed enteral nutrition group, the patients in early nutrition group displayed more reasonable average blood glucose after seven days of enteral nutrition, shorter the duration of insulin use and lower the occurrence of hypoglycemia (P<0.05). Meanwhile, the incidence of pulmonary infection was also lower in early enteral nutrition group than in delayed enteral nutrition group (P<0.05). There were no statistical significance in gastrointestinal hemorrhage, ventilation time and 28-day mortality. Conclusion〓The present findings indicate the early enteral nutrition can effectively improve the stress hyperglycemia and the hospital infection after severe head injury, but not bring beneficial effects on prognosis.

    【Abstract】 Objective To investigate the effects of pancreaticojejunostomy with a pancreatic duct plastic operation during pancreaticoduodenectomy on prevention of pancreatic fistula. Methods From January 2012 to December 2012,.27 patients underwent pancreaticoduodenectomy with pancreatic duct plastic operation without any stent for internal or external drainage for pancreaticojejunostomy. Results All 27 patients were performed the pancreatic duct plastic operation and the operations were completed successfully..The inner diameter of the main pancreatic duct ranged from 2.5 mm to 8.0 mm. According to the International and domestic diagnostic criteria pancreatic fistula,.the pancreatic fistula occurred in 2 cases (Grade A)..The postoperative hospital length of stay was 10 to 14 days. Conclusion The pancreatic duct plastic operation with pancreaticojejunostomy without any stent for internal or external drainage simplifys operation procedure for pancreaticoduodenectomy,.and it has a low pancreatic fistula, and might shorten postoperative hospitalization time.

    【Abstract】Objective To investigate the clinical and diagnostic characteristics of intraductal papilloma of breast. Methods The clinical data of 663 patients with intraductal papilloma from Jan 2008 to Dec 2010 were analyzed retrospectively. Results The age of this group mainly distributed in 30-50 years old and nipple-discharge was the main clinical characteristic. Some cases were non-palpable breast mass and were detected by ultrasonography and vacuum assisted biopsy. In this group,.there were about 40.2% patients who had positive results in ultrasonography and there were only about 10% patients who had a positive results in mammography. The diagnostic accordance rate of fibro-optic ductoscopy for IP was 98%. The sensitivity and specificity of thinprep cytology test (TCT) for IP were 74% and 96.3% respectively. The average follow-up time of this group was 13.3 months, there were 25 patients who had homolateral recurrence and all of these were multiple intraductal papilloma. Conclusions The main clinical characteristic of intraductal papilloma is nipple discharge and fibro-optic ductoscopy is a preferred diagnostic method for intraductal papilloma..TCT is helpful to increase the diagnostic accuracy. Multiple intraductal papilloma has recurrence rate of 4.5%, so the regular follow-up is necessary.

    【Abstract】 Objective To summarize the experience of open window thoracotomy (OWT) in treating tuberculous empyema associated with bronchopleural fistula. Methods Fifty-six cases of tuberculous empyema with bronchopleural fistula were treated by open window thoracotomy. After 3-12 months of the thoracostomy drainage, the patients underwent operation as follows: Heller thoracoplasty with fistula repair, extrapleural total pneumonectomy resection, residual pneumonectomy, permanent open thoracostomy. Results After an effective control of pleural infection,.36 cases were treated by Heller thoracoplasty with fistula repair,.14 cases were treated by extrapleural pneumonectomy or remaining pulmonary resection and then thoracostomies were closed, in which chest infection occurred again in 5 cases during perioperative period and underwent another thoracostomy,6 patients were treated by permanent open thoracostomy window change dressing and drainage.During the perioperative period,.no death case occurred. Conclusion For the tuberculous empyema associated with bronchopleural fistula,.open window thoracostomy combined with antituberculotic therapy would effectively control pleural infection,.reduce mortality,.and also provide a favorable condition for repairing of fistula and eradication of residual cavity to improve the successful rate of the surgery.

    【Abstract】 Objective To improve the knowlege about the clinical features and magnetic resonance imaging findings of idiopathic granulomatous mastitis and avoid misdiagnosis.. Methods   The clinical data,laboratory results,ultrasound and magnetic resonance imaging findings were reported in one case with idiopathic granulomatous mastitis which was misdiagnosed by magnetic resonance and the relative literatures were reviewed. Results The patient underwent segmental excision after blood roatine test,.ultrasound and magnetic resonance and recovered well after surgery. Conclusion pre-operation diagnosis of idiopathic granulomatous mastitis with magnetic resonance is difficult, but it has a definate specific feature in MR finding..If there are correlated sign appearance, the diagnosis of this disease should be considered and it would contribute to provide a therapeutic project for patients.

Results The space before and behind the body of the pancreas and its extension are two major surgical planes. The fusion fascia and fascia space behinal the pancreas were the natural plane of laparoscopic surgical operation. Any deviation whould result injury of nearby blood vessels and organs..LPD along the surgical plane, especially the using of Kocher incision approach,.could fully embody the operation principle of no damage and a radical cure in cancer. The important blood vessels supplying pancreas duodenum were located in the fascia and pancreas parenchyma,.which should be paid more attention to protect the blood vessels near by pancreas duodenal and maintain the integrity of fascia in LPD. Conclusion Understanding of related LPD anatomy characteristics of blood vessels and fascia can provide morphologic basis for laparoscopic surgery in the design and specification of LPD, so as to improve the safety of laparoscopic operation and completeness of radical cure.

    Implant double tissue expanders superposingly in mastoid region for total ear reconstruction.

    Objective To present our clinical experience in using two tissue expanders implanted superposingly in the mastoid region and Medpor ear framework for congenital microtia reconstruction. Methods During the period from Jan 2008 to Dec 2011, 27 patients were treated by total ear reconstruction. In the first surgical stage, a 50 mL and 80 mL (or 100 mL) of kidney-shaped tissue expanders were implanted subcutaneously in mastoid region and then overlaid the big expander on the small one. After the first surgery stage they were inflated with normal saline alternately for 3 to 6 months until the tissue expander was removed in the second surgical stage. After that the anterior and posterior wall of the Medpor ear framework was directly covered by expanded skin flap. A drainage tube was introduced into the space between the expanded skin flap and the ear framework to maintain a healthy contact between them and the contour of the reconstructed auricle. Results The postoperative follow-up time ranged from 4 to 50 months. 27 patients acquired symmetrical, subtle contour, prominent reconstructed auricles. The new reconstructed ear had a good contour and nearly normal skin. The postauricular sulcus was stable and the expanded skin flap looked well without necrosis. The Medpor ear framework was exposed in 2 patients (7.4%) because of wound dehiscence and he healed uneventfully after a further repair surgery. Conclusion For this operation, overlying implantation of double tissue expanders solved the problem of insufficient amount of the skin flap, avoided postauriclar skin grafting and application of the temporoparietal fascia and postauriclar fascia, simplified the surgical procedure and shortened the operation time significantly. The subunit of some reconstructed ear was unclear, but this method gets reliable and satisfactory results.
