    Thrombophilia refers to a pathological condition where the risk of thrombosis is increased due to various genetic or acquired factors. The main clinical outcome of thrombophilia is venous thromboembolism (VTE), which can manifest in various forms such as deep vein thrombosis (DVT), pulmonary embolism (PE), cerebral venous thrombosis (CVT), portal vein thrombosis (PVT), and mesenteric vein thrombosis (MVT). Moreover, thrombophilia also can cause arterial thrombotic events like early-onset acute coronary syndrome and ischemic stroke. Thrombophilia is typically categorized into two main types,genetic and acquired type. Genetic thrombophilia results from inherent defects in the body's anticoagulation or coagulation system, leading to a hypercoagulable state. Patients with thrombophilia often develop thrombotic condition at a young age, and once it occurs, it can lead to severe consequences. In order to enhance the diagnostic and treatment capabilities of clinical doctors in Guangdong Province and provide guidance and standardization for the diagnosis and treatment of thrombophilia at various levels of hospitals, a multidisciplinary team (MDT) of experts in the field of thrombosis was formed. After extensive discussions and seeking opinions from various sources, this consensus document was developed. It primarily focuses on the diagnosis and treatment of genetic thrombophilia and special population prone to clot formation, aiming to provide guidance for clinical decision-making in the management of thrombophilia. This consensus will be regularly revised and updated based on advances in domestic and international research.

    Single-cell profiling reveals that chemotherapy potentiates antitumor immunity in breast cancer through spermidine metabolism modulation

    Objective To explore the alterations of spermidine metabolism in breast cancer in response to chemotherapy, and the subsequent impact on immune cell infiltration and lymphocytic activation. Methods The single-cell RNA sequencing data of MMTV-Wnt1 tumor transplants from the online database were re-analyzed to identify the adjustments of spermidine metabolism in breast cancer cells after chemotherapy. Additionally, investigations were performed on the single-cell RNA-Seq data on multiple breast samples of various subtypes from GEO: GSE176078 to uncover the heterogeneity of spermidine metabolism in cancer cells. Based on the expression of genes associated with spermidine metabolism, breast cancer samples were divided into two categories: the high-spermidine-metabolism group and the low-spermidine-metabolism group. Then the variations in immune cell infiltration and lymphocytic activation were further evaluated between the two groups. Results A subpopulation of tumor cells with active spermidine metabolism and high levels of ODC1 and SMOX expression, the enzymes related to spermidine metabolism, were significantly enriched in the transplant tumors after chemotherapy with doxorubicin. Moreover, active spermidine metabolism in cancer cells was positively correlated with increased effector T-cell infiltration and enhanced CD8 T-cell functions in breast cancer. Conclusion Chemotherapy potentiated antitumor immunity in breast cancer by activating the spermidine metabolism.

    Pancreatic cancer is a relatively common malignant tumor in hepatobiliary and pancreatic surgery, with insidious onset, atypical early symptoms, easy to metastasize, and a five-year survival rate of less than 6%. Pancreaticoduodenectomy is the standard procedure for pancreatic and peripelvic tumors, and laparoscopic pancreaticoduodenectomy has higher requirements for the operator's luminal technique, and more precise requirements for the assessment of tumor invasion of adjacent organs and vascular variations. Three-dimensional visualization technology integrates digital medicine, computer and artificial intelligence technologies, and the three-dimensional visualization platform can reorganize and integrate two-dimensional image data to construct three-dimensional abdominal anatomical structure images, and the three-dimensional display of the anatomical structure around the pancreas is conducive to improving the accuracy of preoperative assessment, increasing the safety of the operation, accelerating the patient's recovery after the operation, and achieving the purpose of precise treatment, which is in line with the accelerated recovery surgery concept. The concept is in line with the accelerated rehabilitation surgery.

    Primary liver cancer(PLC) is one of the most common malignant tumors that seriously threaten the health of Chinese. In China, PLC is currently the second most prevalent cause of tumor death and the fourth most common type of malignancy, with 75% ~ 85% of cases being hepatocellular carcinoma (HCC). Since 2023, the American National Comprehensive Cancer Network (NCCN) Hepatobiliary Duct Clinical Practice Guidelines have been published in a new layout, describing HCC and cholangiocarcinoma separately, which have distinctly different biological behaviors. The 2023 V1 edition of the NCCN Clinical Practice Guidelines for Hepatobiliary Carcinoma (hereafter referred to as the new edition of the guidelines) was first published on March 10. The new edition of the guidelines has more updates than the 2022 V5 edition of the NCCN Clinical Practice Guidelines for Hepatobiliary Ductal Carcinoma regarding HCC, covering more than ten important topics such as screening, diagnosis, and treatment principles, which are of significant reference for China and detailed in this article.

    Objective To explore the application of Sleeping Beauty transposon in screening lung cancer related genes. Methods The Sleeping Beauty transposon was used to insert T2/Onc cassette to mutate HBEC (human bronchial epithelial cells). The positive clones were selected and the CIS (common insertion sites) and candidate genes were screened by high-throughput sequencing. The function of COL11A1 was verified in non-small cell lung cancer (NSCLC) cell line NCI-H1299. CCK-8 cell proliferation and other assays were used to detect the proliferation ability and cisplatin sensitivity of NCI-H1299 cell line before and after knocking down COL11A1. Transwell migration and invasion assays were used to compare the migration and invasion ability of them. Results CIS were determined by Monte Carlo Simulation, which is commonly used in transposon screening. A total of 252 CIS were found with P<0.05 as the bound, including 675 candidate genes. The COL11A1 can promote the proliferation, migration and invasion of non-small cell lung cancer (NSCLC) cell line NCI-H1299 and mediate cisplatin resistance. Conclusion This study demonstrated that the model of mutagenesis of HBEC by SB transposon insertion into T2/Onc can well screen lung cancer-related genes, and confirmed that COL11A1 could promote the proliferation, migration and invasion of NSCLC and mediate cisplatin resistance and epithelial-mesenchymal transition.

    Objective The interaction proteins of U1 snRNA in tumor cells were systematically identified by CHIRP-MS technology. We established a high-throughput interaction protein map of U1 snRNA to study the new biological function and molecular regulation mechanism of U1 snRNA in tumor cells.Methods The biotin-labeled U1 snRNA probe captured the interaction protein binding with U1 snRNA, and some samples were taken to purify the protein and RNA. First, the enrichment effect of the probe was confirmed by qPCR and gel silver staining. Then the obtained protein was identified by high-performance liquid chromatography-mass spectrometry. Finally, the GO, KEGG, protein complex enrichment analysisand PPI analysis were performed by bioinformatics. Results We have identified 135 highly reliable U1 snRNA binding proteins, including 60 known and 75 new U1 snRNA binding proteins. Bioinformatics analysis, such as pathway enrichment, showed that U1 snRNA coordinated and regulated multiple biological processes such as RNA splicing, intracellular transport, localization, telomere, and epigenetics. In addition, the protein-protein interaction network analysis revealed that tumor-related proteins such as ESWR1 may mediate the tumor regulatory function of U1 snRNA. Conclusion In this study, the U1 snRNA interaction protein map was established for the first time in the world, revealing the multiple biological functions of U1 snRNA in tumor cellsand providing a theoretical basis for further study of U1 snRNA's involvement in tumor development, prevention, and treatment.

    Most patients with hepatocellular carcinoma are diagnosed at intermediate-advanced stage in China, losing the chance of long-term survival through surgical resection. In recent years, locoregional therapy and systemic therapies of hepatocellular carcinoma have achieved great progress and more patients with intermediate-advanced unresectable hepatocellular carcinoma are expected to benefit from conversion therapies. After preliminary exploration, the multi-mode and high-intensity conversion therapies through combining locoregional therapy and system therapy has shown a gratifying success rate. However, there is still no consensus on the target population, regimen options and operation opportunity of conversion therapies. The paper reviews the domestic and foreign research progress and discusses several focus issues of conversion therapies for intermediate-advanced hepatocellular carcinoma.

    Background A growing number of Chinese breast cancer patients are diagnosed at a young age. The quality of life of young breast cancer patients has been a critical issue. Breast-conserving surgery (BCS) not only removes the tumor but also maintains the appearance of breast. The Breast Tumor Center of Sun Yat-sen Memorial Hospital is one of the first departments in China to perform breast-conserving surgeries. Endoscopic breast surgery has emerged as a promising surgical approach. However, it is hard to delineate the tumor margins in endoscopic BCS, which restrains its development. In traditional BCS, surgeons determine the tumor border by palpation, which is impossible in endoscopic BCS. For the first time, we performed the intra-operative navigation system-assisted endoscopic breast-conserving surgery, in which the tumor border was accurately delineated using the navigation system. Patient A 46-year-old woman was diagnosed with left breast cancer with the stage of cT2N1M0 IIB. She asked the surgeons to maintain the breast appearance to the greatest extent. Methods Breast MR was performed to acquire the three-dimensional images. At the start of surgery, the spatial position of breast was acquired by scanning with O-arm device after skin preparation and draping. Then, we integrated the images and breast position in a real-time 3-dimensional model of Medtronic Stealth Station S7 Navigation System with the “3D Fusion” function. Finally, we used the probe to precisely mark the tumor border and excised the lump. Results The margins of the tumor were all negative in pathological assessment. The breasts looked basically symmetrical and the wound was invisible when viewed from the front. The patient was pleased with the appearance. Discussion Intra-operative navigation system-assisted endoscopic breast-conserving surgery achieves an easier operative procedure and a lower positive rate of surgical margin, which may vigorously promote the endoscopic BCS.

    Intrahepatic cholangiocarcinoma is a highly invasive malignant tumor. In recent years, the incidence has been increasing every year, and radical resection is considered as the only effective way to cure ICC. Lymph node metastasis is an important prognostic risk factor for ICC. In the radical resection of intrahepatic cholangiocarcinoma, the issue of lymph node dissection remains controversial, in which whether lymph node dissection, the way and extent of lymph node dissection, as well as the significance and value of lymph node dissection are the focus of the current research. In this paper, the hotspots and controversies of lymph node dissection for intrahepatic cholangiocarcinoma were discussed.

    The drug treatment of metastatic renal cell carcinoma is the focus at present. The 2020 edition of EAU guidelines of renal cell carcinoma recognizes the role of Cabozantinib for the first time and lists it as the first-line treatment for patients who cannot accept immune checkpoint inhibition treatment. This paper will elaborate the treatment mechanism, treatment scheme, treatment effect, adverse events and treatment cost of Cabozantinib, and analyze the advantages and disadvantages of Cabozantinib in detail. It will also compare Cabozantinib with sunitinib, and further explore the prospect of Cabozantinib for metastatic renal cell carcinoma.

    Objective To introduce how to identify intrahepatic anatomical structure by laparoscopic ultrasound (LUS). Methods In one case of laparoscopic hepatectomy, using LUS to complete liver map scanning through different Trocars was performed and its imaging was collected. Results Several standard imaging of intrahepatic anatomical structure were obtained by axial rotation scanning of LUS: ①Right hepatic pedicle, right hepatic vein and middle hepatic vein were scanned by LUS through a subxiphoid Trocar. The right hepatic pedicle branched out into AT and PP, and then branched out into P5, P8 and P6, P7; V5d, V6, V8d and AFV flowed into the right hepatic vein successively, while V4, V5v and V8v flowed into the middle hepatic vein. ②Left hepatic pedicle, left hepatic vein, and middle hepatic vein were scanned by LUS through a trocar under the right costal margin. The left hepatic pedicle branched out into P2, P3, P4, V2, V3 flowed into left hepatic vein, and inferior vena cava and the vertical axis of middle hepatic vein were also scanned from this way. Conclusion The “standard imaging” of LUS in liver scaning is very helpful to identify the intrahepatic anatomical structure correctly. Surgeons must master the essential technique of LUS in liver scaning, which needs in-depth study and repetition training. After that, LUS would play a guiding role in laparoscopic hepatectomy.

    The incidence of breast cancer is increasing year by year. Compared with other organ metastasis, the prognosis of breast cancer patients with liver metastases is usually poor. At present, there is no guideline for diagnosis and treatment of liver metastasis in breast cancer patients. The purpose of this paper is to provide ideas and suggestions for clinical diagnosis and treatment of breast cancer with liver metastases. Reviewed the clinical features, pathogenesis and therapy ofbreast cancer patients with liver metastases. When liver metastasis occurs, patients usually had no obvious symptoms or signs.It is strongly recommended thatliver metastasis should be pathologically diagnosed by percutaneous liver puncture. With the development of new drugs and therapies for breast cancer, the survival rate of patients with liver metastases has improved significantly, and some patients can also benefit from local treatment. Effective systemic therapy is the key to improve the survival of breast cancer patients with liver metastases. Some selected patients may also benefit from local treatment

    Biliary tract cancer has insidious onset and rapid development thus most patients have lost the opportunity of surgery when they are found, so the prognosis is poorer. Conversion therapy can achieve the goal of making the tumor shrink or reduce its stage, so that patients have conditions for surgery and complete radical resection. Due to the occurrence and development of biliary tract cancer with great heterogeneity is the result of multiple elements and paths, combination therapy is becoming the main direction to improve the success rate of translational therapy and the prognosis currently. With the deepening understanding of the pathogenesis and development of biliary tract cancer, individualized and precise combination therapy will become the main direction of conversion therapy in the future.

    The incidence of pancreatic cancer is increasing quickly, with dismal prognosis. In order to improve the long-term survival of pancreatic cancer, it is necessary to employ the disease-centered multidisciplinary model in the diagnosis and treatment. Via effective combination of multidisciplinary team to achieve the whole journey management of diagnosis, staging, precision medicine, rational operation, and supportive care. Improvement of long-term survival could be accomplished as new advances of multi-omics, tumor immunology, target therapy and chemo-radiation therapy.

    Colorectal cancer is the second most common malignant tumor in China, and about 50% of patients will have liver metastases. Effective systemic treatment and active local treatment including surgery are the key to improving the efficacy of patients with colorectal liver metastasis. Recurrence after hepatectomy is the main cause of death in patients with colorectal liver metastasis. The treatment options for patients with different recurrence risks are also different. Therefore, it is very important to establish an effective prognostic scoring system for colorectal liver metastasis to guide its individualized treatment. Clinical Risk Score (CRS) is currently the most widely used prognostic scoring system for patients with colorectal liver metastasis. Other prognostic scoring systems include Tumor Burden Score (TBS), Genetic And Morphological Evaluation (GAME) score, and Comprehensive Evaluation of Relapse Risk (CERR) score. The continuous improvement of the prognosis scoring system can better help clinicians predict the prognosis of patients with colorectal liver metastasis, and adopt more active intervention methods for high-risk patients to reasonably and effectively prolong the lives of patients.
