    Expression and signification of NF-кB and MUC1 in intrahepatic lithiasis associated with intrahepatic cholangiocarcinoma

    Objective To investigate the expression and significance of NF-кB and MUC1 in hepatolithiasis associated with intrahepatic cholangiocarcinoma. Methods Ninety cases of liver tissues specimen of hepatectomy were selected in The Second Affiliated Hospital of Sun Yat-sen University in recent 20 years. Among them, 33 cases with hepatolithiasis associated with intrahepatic cholangiocarcinoma were as experiment group, 32 cases with hepatolithiasis were as control group and 25 cases with normal intrahepatic duct tissues were an blank group. The immunohistochemical staining of NF-кB and MUC1 were performed by tissues slice.Results 1)There was significant difference in NF-кB and MUC1 positive expression rate (P<0.05).2)MUC1 positive expression rate showed significant difference in histopathologic grading and in lymph node metastasis (P<0.01,0.05).3)Accumulated survival rate of MUC1 expressed tumor patients was lower than that of MUC1 non-expressed tumor patients(P<0.01). Conclusion NF-кB expression is probably the early stage in the carcinogenesis of intrahepatic cholangiocarcinoma and in the formation of hepatolithiasis. High level expression of MUC1 is related with malignan degree of cholangiocarcinoma and with worse prognosis.
    Clinical significance of endoscopic biliary drainage in obstructive jaundice: Report of 95 cases

    Objective To evaluate the clinical significance of endoscopic biliary drainage in treatment of obstructive jaundice.Methods: The clinical records of biliary drainages under duodenoscope in 95patients with biliary obstruction from October 2003 to November 2008 was reviewed retrospectively . Results :Drainage was successfully completed in 91patients,with a success rate of 95.8%(91/95).The patients were relieved of their symptoms of biliary obstruction(abdominal pain,jaundice and fever),and the effective rates of ENBD 、ERBD and EMBE were 93.5% (58/62)、 93.5% (29/31) and100% (2/2),respectively .The patients with malignant biliary obstruction survived for an average of 10.2 months(3 to 20).Conclusions : Endoscopic drainage for biliary obstruction has advantages of minimal invasion,rapid recovery,short hospital stay and apparent efficacy.Endoscopic biliary drainage is a safe and effective method for bile duct obstruction,It Can prolong the survival time and improve quality of life in patients with malignant biliary obstruction.

    Objective To observe concentration changes and intervention of somatostatin of MMP-2 and TIMP-2 of plasma after hepatectomy in patients with liver cancer. Methods Twenty cases received hepatectomy were randomly divided into teo groups namely conventional therapeutic group and somatostatin treatment group. Ten cases were in each group. The somatostatin was given in the somatostatin treatment group. Results The plasma concentration of MMP-2 was significantly increased and The plasma concentration of TIMP-2 was significantly decreased after hepatectomy. The administration of somatostatin could decrease the plasma concentration of MMP-2 and increase the plasma concentration of MMP-2. Conclusions The administration of somatostatin after hepatectomy might suppress of recurrence after surgery.

    Objective To evaluate the significance and method of preoperative prediction of the difficulty of laparoscopic cholecystectomy . Methods The preoperative clinical data was collected from 500 cases underwent LC. According to the standard of operative difficulty or ease, the 500 cases with gall bladder diseases were divided into two groups, namely difficult group and easy group.All the preoperative and operative data were compared between the two groups. Results The accuracy rate of preoperative diagnosis with sonography for cholecystolitiasis were 98.61% .465 cases were performed with LC,; 35 cases are required to convert into open cholecystectomy. The conversion rate was 7%. The total prediction accuracy of sonography was 92.4% and the sensitivity was 96.41% ,; specificity was 78.19% . Conclusions ① The diagnosis with B--type ultrasound shows high accuracy rate for the diagnosis of the disease of gallbladder , and it has a definite value for the prediction of the difficulty of LC .② Severe adhesions presented in Calot’s triangle or gallbladder , incarceration with stone at gallbladder’s neck associated with acute cholecystitis , the repletion type of cholecystolithiasis , atrophic cholecystitis and previous surgery at the upper abdomen have an important value for the evaluation of the difficultiy of the LC.

    Objective To investigate the clinical effects on Millagan-Morgan procedure combined with SETUS-M in the treatment of Ⅲ°and Ⅳ°mixed hemorrhoids. Methods From January 2006 to January 2009,180 cases with Ⅲ°and Ⅳ°mixed hemorrhoids were randomly divided into two groups,namely experimental group and control group. The Millagan-Morgan procedure combined with SETUS-M were used in 90 cases of experimental group. Only Millagan-Morgan procedure was used in 90 cases of control group. The observation and comparison were performed between two groups, including postoperative painful time, frequency in the use of analgetic, postoperative bleeding, swelling time of surface of wound, complications, hospitalization time,recovering normal time of work and life and adverse reaction of drug. Results In the comparidon between experimental group and control group, the former was shorter than that latter in the postoperative painful time, swelling time of surface of wound, hospitalization time and recovering normal time of surface of wound(P<0.05), postoperative bleeding was decreased significantly(P<0.01) and postoperative complications were declined significantly(P<0.05).But the postoperative urinary retension was no obvious difference(P>0.05).In the experimental group, no adverse reaction was found in taking SETUS-M period. Conclusion Millagan-Morgan procedure combined with SETUS-M is a cheap, safe,and effective therapeutic method for the treatment of Ⅲ°and Ⅳ°mixed hemorrhoids.

    Objective To investigate the method and clinical value of preserving intercostobrachial nerve(ICBN) during breast cancer operations Methods 165 cases with breast cancer underwent operation. Among them, 102 patients had intercostobrachial nerve reservation and the nerve was resected in 63 cases.All patients were followed up for 6 to 36 months. Results Among the 102 patients with ICBN reservation,sensory disturbance of affected upper medial arm and axilla was 8.8% (9/102);in ICBN resected group,the incidence was 95.2%(60/63),P<0.001.All cases were followed up without local recurrence. Conclusion Preservation of ICBN were effectively reduce incidence in the sensory disturbance of the affected upper medial arm and axilla and improves the patient’s quality of life after breast cancer operations.

    【Abstract】 Objective To analyzed the recurrent causes of nodular goiter after operation and management. Method The data of 45 patients who sufferred from recurrent nodular goiter after operation between 1998-2008 were reviewed. Results The recurrent nodular goiter was related to the type of disease, surgical procedure and irregular TSH inhibition after operation. There was no recurrent case when 45 patients underwent reoperation and were treated with thyroxin. Conclusion Full examination before operation and careful exploration during operation shoud be done in first resection. Suitable surgical procedure and treatment with thyroxin after operation can decrease the recurrent rate.
    Strategy of Warfarin anti-coagulation after Mechanical Heart Valve Replacement

    Objective To study the strategy of warfarin anticoagulation and the relationship between age, gender, body weight and the coagulation dose,efficacy of warfarin, after mechanical heart valve replacement. Methods From September 2006 to September 2008, the clinical data of 192 cases received mechanical heart valve replacement and used warfarin anticoagulation therapy after operation were analyzed retrospectively. Results In this group, because of warfarin used overdose, 1 case died from intracranial bleeding and 1 case occurred extensive subcutaneous bleeding and severe nasal bleeding, 1 case occurred luminal space cerebral infraction (only single use of warfarin). 2 cases were ineffective in the only use of warfarin, but after combination with aspirin, the INR could adjust to the target range. The other cases had a good result, no severe bleeding and thrombosis were found. Conclusion Maintain dose of warfarin anticoagulation has a great individual difference after mechanical heart valve replacement. It not correlated with age, gender and body weight. To maintain anticoagulation therapy INR at 1.8 to 2.5 is appropriate, which dose not increase the risk of bleeding and thrombosis.

    Objective To discuss the operative method and clinical effect of pneumonectomy plus partial resection of left atrium in locally advanced lung cancer. Methods From Dec. 2001 to Dec. 2006, 16 patients with locally advanced lung cancer involving left atrium received partial resection of left atrium plus pneumonectomy were analyzed retrospectively.In these cases, 6 underwent the left lower lobe resection, 4 underwent left pneumonectomy, 3 underwent right and lower lobe resection, 3 underwent right middle lower lobe resection. Results No operative death or intraoperative complication was found. Arrhythmia happened in 2 cases, pneumonia in 1 case. The average survival period was 36 months. The postoperative survival rates of 1,3,5- years were in 75%(12/16)、43.75%(7/16)、25%(4/16)retrospectively. Conclusion Pneumonectomy combined with partial resection of left atrium and postoperative chemotherapy have a good clinical effect, and would improve the patients’survival quality and prolong the survival time.

    Objective To investigate the efficacy of mini-insision thyroidectomy with FOCUS ultrasonic scalpel. Methods Mini-incision thyroidectomy with FOCUS ultrasonic scalpel was performed in 80 cases with thyroid diseases from 2008 July to 2009 September. Results The mean operation time was 45 min, ranging from 30 to 75 minutes. The mean blood loss was 15 ml, ranging from 5 to 30 ml. No postoperative hoarse voice, secondary bleeding, low calcium convulsion were observed. The average drainage of 24 hours in postoperation was 20 ml, ranging from 15 to 30 ml, and the average hospital stay in postoperation was 3 days, ranging from 2 to 5 days. Conclusions FOCUS ultrasonic scalpel for thyroidectomy can reduce the volume of bleeding, complication, recovery time and operation time, and possesses a good cosmetic outcome and less trauma.

    Objective To investigate the safety and clinical value of (LPC) without dissecting Calot triangle in decreasing conversion rate of difficult cases. Method The clinical data of 43 cases received laparoscopic subtotal cholecystectomy without Calot triangle were analyzed retrospectively. Results Among them, 8 cases were diagnosed as atrophic cholecystitis, 32 chronic cholecystitis with acute attack, 3 acute cholecystitis. No bile was drainaged in 29 cases, Abdominal drainage were removed within 5 days after operation in 10 cases, 7 days in 3 and 14 days in 1. All cases were cured to discharge from hospital.Follow up for at least 6 months ,no complications were found . Conclusions This procedure is safe and can effectively decrease the rate of open operation in the use of cases of difficult laparoscopic cholecystitis.
    Repair of preperitoneal Kugel patch via anterior approach in the treatment of inguinal hernia

    Objective To investigate the therapeutic efficacy on the rapair of preperitoneal Kugel patch for repairing inguinal hernia with preperitoneal Kugel patch of anterior approach. Methods From Apr. 2004 to May 2005,the clinical date of 25 cases with inguinal heria received repair of preperitoneal Kugel patch via anterior approach were analyzed retrospectively. Results The mean operative time was 40 minutes(35-55 minutes). All cases could left the bed in 6-12 hours after operation. No postoperative urinary retentions, incisional hematomaand infection were found. With postoperative follow-up time for 1-12-month,no recurrence was found. Conclusion Repair of inguinal hernia using preperitoneal Kugel patch of anterior approach is good ideal procedure. It has minimally invasive, tension-free ,safe and postoperative rapid recovery.
    Differential diagnosis of ureteral space occupying lesions by multi-slice spiral CT

    Objective To investigate the applied value of multi-slice spiral CT in the differential diagnosis of ureteral space occupying lesions. Methods The CT data of 16 patients with ureteral carcinoma ,4 patients with ureteral endometriosis and 3 patients with ureteral polyp which confirmed by pathology were retrospectively analyzed. Results CT findings of 16 cases of ureteral carcinoma were ureteral wall thickening(3 cases),intraluminal filling defect(7 case),extraluminal mass(6 cases)and retroperitoneal enlarged lymph node(5 cases). CT findings of 4 cases of ureteral endometriosis were extraluminal mass(3 cases),intraluminal filling defect(1 cases),and combined with ovarian cyst(2 cases). CT findings of ureteral polyp were intraluminal filling defect in 3 cases. Conclusion multi-slice spiral CT has a high value in differential diagnosis of ureteral space occupying lesions.

    Objective To research the clinical efficacy of tamsulosin in treatmeng of patients with lower ureteral stones..Methods A total of 82 patients with lower ureteral stones were randomly divided into 2 groups. In the group A, shilingtong was taken by oral with 5 tablets one time and three times a day. In the group B, shilingtong plus tamsulosion were taken by oral and the latter dosage was 0.4 mg one time a day. The lithagogue rate mean lithagogue time and cases of used analgesics were observed. Results In the lithagogue rate, mean lithagogue time and used rate of analgesic hasd significant difference between two groups(P<0.05). Conclusions With troditional lithagogue plus tamsulosion lithagogue treatment, it would elevate lithagogue rate, shorten lithagogue time and reduce incidence of renal colic for low ureteral stones.

    Objective To explore the effect of minimally invasive percutaneous nephrolithotrips(MPCNL) on stress response of patients with renal calculus . Methods we retrospectively analyzed 82 cases of renal calculus from February,2004 to October,2008 in our hospital with 46 with MPCNL (MG) and 36 with open operation (OG) . peripheral blood samples were obtained preoperatively and 3 h, 24 h after operation for measuring cortisol, IL-6, C-response protein and glucose. Result The levels of cortisol, IL-6, CRP and glucose in both OG and MG were remarkably higher at 3 h postoperatively than that at preoperation, The levels of cortisol, IL-6, CRP and glucose in the OG was much superior to the MG (P<0.05). Conclusion The results of this study indicate that minimally invasive percutaneous nephrolithotrips imposes less influence on stress response of patients with renal calculus than the OG.This might suggest the quick recovery of the patients receiving MPCNL.

    Objective  To study the effect of vertebral canal reconstruction with micro titanium plate after multi-cervical segments Intramedullary tumor resection. Methods The enbloc laminectomy was performed by Tomita method in the 24 cases with multi-cervical segments intramedullary tumor. Among them, 7 cases resected laminae were re-implantation with micro titanium plate fixation. The vertebral canal were reconstructed and spinal stabilities were kept. Results No vertebral canal stegnosis, kyphosis and spine unstability were found after laminal re-implantation and vertebral canal plasty. However, the kyphosis, respiratory disturbance, sphincter dysfunction, cerebral spinal fluid leakage associated with intraeranial infection and epidural hydrops were in 5,2,5,5 and 3 cases respectively without laminae re-implantation and vertebral canal plasty.Conclusion After Tomita enbloc laminectomy, micro titanium plate fixed laminae re-implantation has keeping postoperative spinal stability for treatment of the patients with multi-cervical segmental intramedullary tumor and it would reduce the occurrence of postoperative complication.

    Locking plate for the treatment of proximal humeral comminuted fracture in the elderly Tan Jiaqun,Li Jianchi, Xie Weijian, et al. Department of Orthopedics, The Guizhou Hospital of Shunde District, Foshan, Guangdong,528305 [Abstract] Objective To investigate the clinical efficacy on the use of locking plate for treating osteoporostic proximal humeral comminuted fracture in the elderly. Methods From January 2007 to January 2009, 26 cases with proximal humeral comminuted fracture received treatment of compressive locking plate internal fixation were analyzed retrospectively. According Neer typing, the fracture of part 3 and part 4 were in 19 and 7 cases respectively. All cases were fresh fracture. Results No wound infection and hydrosy and loose ness of fixation plate were found after operation. 24 cases were followed-up for 9-24 months after operation. The union was good in all cases. No humeral ischemic necrosis head was found. According Neer functional score, the excellent and good rate were in 83.33%. Conclusion Locking compressive plate possesses reliable fixation and satisfactory therapeutic efficacy for the treatment of proximal humeral comminuted fracture in the elderly.
    Clinical study on the compound material of nanometer hydroxyapatite in the treatment of long bone comminuted fracture

    Objective To compare the therapeutic efficacy between nanometer material and auto-bone graft in the treatment of long bone comminuted. Methods From June 2006 and July 2008, 71 cases with long bone comminuted fracture received surgical operation were analyzed retrospectively. Among them, 37 cases were treated with auto-bone graft and internal fixation, and 34 cases were treated with nano-material graft and internal fixation.Results All cases were followed up respectively over 12months. In the cases of auto-bone graft, the healing rates were in 34.9%、69.8% and 97.7% within 3, 6 and 12 months, While in the cases of nano-material graft, the healing rates were in 32.4%、67.6% and 97.3%, respectively within 3, 6and 12 months. No significant differences in the bone healing and joint function were found between two groups. Conslusion:Nanometer hydroxyapatite would use in the treatment the long bone comminuted fracture and gained well bone healing.

    Objective To investigate the application and curative effect on paoximal femoral nail (PFN) in the treatment of intertrochanteric fracture of femur. Methods From April 2004 to August 2009, 48 cases with intertrochanteric fracture of femur received treatment of closed reduction and OFN internal fixation were analyzed retrospectively. According to Evans-Jensen typing criterion of intertrochanteric fracture of femur, the type Ⅰ,Ⅱ,ⅢA,ⅢB AND Ⅳ were in 2,6,7,5 and 3 cases respectigely. Roentgenograph was re-checked to judge the union good rates of articular function were evaluated. Results No death was found during hospitalization. Except for 2 cases died from cardiovascular systemic diseases, follow-up was got in 21 cases. The follow-up time ranged from 9-15 months(mean 13 months). The union was got in all cases. The postoperative hip joint function score: the excellent, good and poor were in 18,2 cases and 1 case respectively. The excellent and good rate were in 95%. Conclusion PFN possesses less trauma;reliable fixation, good anti-rotated action and less postoperative complications. It is an ideal method of internal fixation for the treatment of intertrochanteric fracture of femur.
    A survey on the incidence of idiopathic scoliosis for primary and secondary school students in Shunde District

    Objective To understand the prevalence of idiopathic seoliosis of primary and secondary school students in Shunde district of Guangdong province and provided basis for clinical treatment and prevention. Methods From March 2007 to December 2009,the screening of idiopathic scoliosis was done in the 13247 cases of primary and secondary school students for 13schools medical and health monitoring points in Shunde District. Results nimety four cases with idiopathic scoliosis were detected(incidence o.71%).Among them, 27 cases were male and 67 cases were female. In the incidence, the male was significantly lower than that female(0.37%vs1.11%,P<0.01).In the incidence, the junior middle school group was significantly higher than those primary school group and senior middle school group(X2 was 13.58 and 16.97 respectively, P<0.01).However, no significant difference was found between primary school group and senior middle school group in the incidence (X2=0.41,P>0.05) Conclusion The incidences of idiopathic scoliosis of primary and secondary school students are 0.71%in Shunde District,and the incidence of female is significantly higher than that male. This changes are specially evident in the junior middle school group.
    Expansive Cervical Laminoplasty by Anterior and Posterior Cervical Decompression in the Treatment of Cervical Spondylosis Myelopathy

    Objective To evaluate the clinical results of expansive laminoplasty by anterior and posterior cervical decompression in the treatment of cervical spondylosis myelopathy. Methods 48 cases of cervical spondylosis myelopathy treated with this method from March 2001 to Februa- ry 2005 were reviewed. Neural functionality (JOA Scoring System), axial cervical symptoms, MRI and dynamic lateral x-ray of cervical vertebrae were evaluated. Results All the 48 cases had been followed-up for an average of 28 moths. The excellent and good rate when followed-up for 3, 6, 12, and 24 months were 75.0%, 81.3%, 85.4%, and 89.6%, respectively. Conclusions The met- hods of expansive cervical laminoplasty by anterior and posterior cervical decompression in the treatment of cervical spondylosis myelopathy could not only decompression thoroughly and keep the stability of cervical vertebrae, but also improve the rate of interbody fusion and maintain the interbody distance. The methods of expansive cervical laminoplasty by anterior and posterior cer- vical decompression in the treatment of cervical spondylosis myelopathy have good efficacy and it is a reliable alternative.

    Objective: To study the CT and MRI manifestations of Spinal Ewing’s sarcoma (ES). Methods: From January 2003 to October 2009, the imaging data of 7 cases with spinal ES confirmed by pathology were analyzed retrospectively. Results: The lesions involved cervical, thoracic and lumbar vertebra were in 3, 3 cases and 1 case respectively. CT examination showed that vertebral body and/or appendix osteolytic bone destruction were in 2 cases. Three cases were intra- and extra-spinal soft tissue mass. MRI examination showed that seven cases manifested as intra- and extra-spinal soft tissue mass. T1-weighted image showed low, equal or slightly high signal in 7cases. T2-weighted image showed uneven high signal and contrast image showed heterogeneous enhancement in 6 cases.Only one case had slightly high signal on T2-weighted image and homogeneous enhancement on contrast image. According to immunohistochemical results, CD99 in all cases were positive. Conclusions: Spinal ES should be considered in adolescent patients with aggressive osteolytic bone destruction of vertebrae and intra- and extra-spinal soft tissue mass. The final diagnosis of spinal ES depends on the pathologic and immunohistochemical determinations. CT and MRI should be accurately and synthetically used on spinal ES, which can give more information to diagnosis, treatments, evaluating curative effects and prognosis.

    Objective To investigate the clinical efficacy on the trophic vessels skin flap of medial dorsal cutaneous nerve of foot in the repair of distal dorsal defect of surface of wound. Methods From January 2008 to May 2009,15 cases with distal dorsal of surface of wound with trophic vessels skin flap of medial dorsal cutaneous nerve of foot in the repair of distal dorsal defect of surface of wound were analyzed retrospectively. Results 15 cases of skin flap were survival totally. The surface of wound was first attention and secondary intention in 13 and 2 cases respectively. The average follow-up time was ranged from 6 months(4~8). The skin flap sin color, natural constitution and quality, thick and thin were similar to recipient area skin. Algesia and thalposis were recovered totally. The patients walking was in normal weight carrying. No ulceration was found in the skin flap and skin of supplying dkin graft area. Conclusion The trophic vessels skin flap medial dorsal cutaneous nerve of foot possesses simplified operative manipulation, safe, reliable blood supply and it is a effective method for the repair of middle and minor area skin and soft tissues defect in the distal dorsum of foot.
    Effect of desmopressin and hemocoagulase on hemostatic function in scoliosis surgery.

    Objective To observe and compare the hemostatic effect of deamino-8-D-arginin vasopressin (DDAVP, desmopressin) and hemocoagulase in scoliosis surgery. Method Twenty-seven patients undergoing posterior reconstructive surgery for scoliosis were randomly assigned to either group A (n=12, hemocoagulase, 2KU) or group B (n=17, DDAVP, 0.3μg/kg). Mean arterial blood pressure (MAP), heart rate (HR) and central venous pressure (CVP) were continually monitored. Activated coagulation time (ACT) was recorded at 15, 30, 60, 120, 180, 240 minutes after transfusion of hemostatic drugs. Total blood loss, total urinary output during surgery and 24hr after surgery were also recorded. Result MAP at 10 minutes after hemostatic drug transfusion in group B was significantly lower than that in group A, P<0.05. There was no statistic deference between ACT data, total blood loss and urinary output in group A and B. Conclusion This study demonstrated that the hemostatic effect of DDAVP and hemocoagulase were similar. DDAVP did not alter the urinary output but caused brief systemic hypotension.

    Objective To analysis the reason, clinical manifestation, diagnosis and therapy of gastroplegia after abdominal surgery. Methods 45 patients with gastroplegia after abdominal surgery, adopted in the recent five years in Sun Yet-sen memorial hospital of Sun Yet-sen university and Huangpu People’s hospital of Zhongshan, were retrospectively analyzed. Result: Clinical manifestation, Gastroscopy and gastroenterography play a major role in the diagnosis of gastroplegia. Gastroplegia usually occurrence in 3~7 days after abdominal surgery. gastrointestinal function can be recovered in 2~14 weeks by systematic conservative treatment. Conclusion: Systematic conservative therapy were helpful to gastroplegia and secondary operation should be avoided.

    Objective To observe of the preventive effect of the cold saline gargling to chemotherapy- induced stomatitis for patients with breast cancer. Methods 157 patients with breast cancer chemotherapy were randomly divided into groups A, B, C .In group A, after the start of chemotherapy, 53 cases gargled with cold saline. In group B, after the start of the chemotherapy,50 cases gargled with xinzuo gargle.In group C, 54 cases had no additional other treatment. All patients were observed after one week for incidence and composition of stomatitis. Results The incidence of stomatitis in groupA, B, C were 5.66%, 18.0%, 25.93% respectively. The comparision between groupA and group B:?2=3.85,p<0.05, the comparision between group B and group C: ?2=1.95,p>0.05, and the comparision between group A and group C: ?2=8.22,p<0.01. Conclusion Cold saline gargling would more effectively prevent the occurrence of stomatitis.
